BigBendHealth.com Signup Process

To signup for BigBendHealth.com Health Information Exchange (HIE) services please complete the following three step process. Additionally you may CLICK HERE TO DOWNLOAD a full copy of the Master Participation Agreement and Terms and Conditions of Use for the system.

	Register Organization Download and complete the Organization Registration Agreement CLICK HERE TO DOWNLOAD. Additional information on HIE Services are also available for your review.
	Authorize Users Download and complete the User Application Form CLICK HERE TO DOWNLOAD for each user within your organization that will require access to the HIE system. Once your organizaiton has completed the registration process there is no limit to the number of individual user accounts that can be provided.
	Setup Training Coordinate setup and training with BigBendHealth.com staff by calling 850-702-0365 and ask for BigBendHealth.com services or emailing contact@bbrhio.com.
